To request records, please contact us by phone or email. To help us locate the correct file, please be prepared to provide the following information:
- Full legal name of the patient
- Date of birth
- Approximate dates of treatment, if known
- Best phone number and email for follow-up
- Whether the request is from the patient, legal guardian, provider, attorney, or authorized representative
- Any signed authorization forms, if applicable
